Lorenzo Alcazar, Luis Alcazar's brother, came to Port Charles in June 2003 to avenge his younger brother Luis' death. Alcazar took Sonny hostage in exchange for drugs. Ric and Jason worked together to set Lorenzo up and ultimately got him arrested for drug-related charges.

Alcazar discovered that Ric kidnapped Carly to steal her unborn baby and blackmailed Ric into working with him. He repeatedly went to visit Carly in the panic room where she tried to get him to help her. Ultimately he admitted to Carly the reason he partnered up with Ric is because he held Sonny responsible for Luis' death.

Alcazar finally realized that Ric was unreliable and that Sonny would make a better business partner. He attempted to blackmail Sonny with information on Carly's whereabouts in exchange for him allowing Alcazar to land shipments in his territory. When this plan failed to work, he decided to steal Carly and use her again as a bargaining chip with Sonny to force him to let Alcazar run drugs through his territory. Alcazar kept Carly in his yacht and they ended up developing a unique friendship with each other. He even let Sonny visit Carly briefly, but when he found out Sonny put a honing device on Carly, Alcazar freaked out and took her to a remote location. Carly went into premature labor and when the doctors were forced to perform an emergency c-section, Carly begged Alcazar not to let them perform the procedure. Alcazar obliged and Carly was grateful to him, but Jason and Sonny were not far behind them and ended up rescuing Carly at the clinic. Meanwhile Courtney followed them to South America and was kidnapped by Alcazar who wanted to trade Courtney for Carly. Courtney escaped into the ocean where she suffered a miscarriage.

Throughout this time, Alcazar had fallen in love with Carly and would stop at nothing to get her in his life. Alcazar learned about Sonny's wife Lily who died in a car bomb meant for Sonny. He hired a hooker and gave her plastic surgery to look like Lily to drive Sonny over the edge.

When Carly went into labor, Alcazar was there to help her deliver the baby. Sonny barged in and found Alcazar with Carly, but he assumed the worst and shot him. Unfortunately the bullet went directly through Alcazar and into Carly's brain. Everyone was rushed to the hospital where eventually baby Morgan was fine and both Carly and Alcazar survived. But Carly had changed and although she remembered her family, she couldn't remember her love for them, including Sonny. She began to have dreams about her "other" life with Alcazar, the history professor. She dreamt of a life that could have been and ultimately she fell in love with him.

Carly's memory of her love for Sonny fully returned, but ultimately it was too late. When Carly walked in on Sonny in bed with Sam McCall, she went directly to Alcazar and made love to him. Alcazar stood by Carly through her custody battle for her children, he even planted a car bomb to make Sonny look bad to the judge and lied to Carly about it. When Carly found out the truth, she broke up with Alcazar and could not forgive him.

Recently, Alcazar has found interest in Lois Cerullo.
