Sage Alcazar is the niece of Lorenzo Alcazar who came to Port Charles to avenge her father, Luis' death. She first came to Port Charles to find Alexis Davis who responsible for her father's death. Looking for some attention, she jumped off the docks where Dillon happened to be there to fish her out of the water. He took her home where she hid under his bed while Georgie told him she wanted to have sex.

She tried to shoot Alexis at Luke Spencer's trial and ended up shooting Dillon in the arm. Sage immediately took a liking to Dillon, but he had a girlfriend, Georgie who couldn't help but be threatened by her. Alcazar hired Dillon to be Sage's bodyguard, angering Georgie even more. Dillon took Sage out of the country to protect her and ended up in jail after Sage got them in trouble. Georgie feared for Dillon's safety and partnered up with Tracy Quartermaine, Dillon's mother. Tracy agreed to force Dillon to quit working for Alcazar if Georgie broke up with Dillon. He was devastated and ended up giving into Sage and losing his virginity to her. To his surprise, it was her first time too.

Sage's Uncle Lorenzo agreed to help finance L&B Records if Lois helped Sage's singing career.
